To Make a Stallion Service Donation
To donate a stallion service, please complete the two forms and return them along with any ancillary documents such as stallion pictures, breeding contracts, progeny records, or performance records electronically to Dr. John Dascanio as attachments to email.
We will include these documents in the stallion's personal page. If you unable to send them electronically please mail or fax to the address listed below. We would prefer electronic submission if possible. If you do not have access to a printer, contact Dr. John Dascanio to request that a donation packet be mailed to you.
The forms are either in PDF format or in Microsoft word. You will need Adobe Acrobat Reader to be installed on your computer to open the PDF formatted files. You only need to fill out one type of format of each form.
